腹腔镜技术应用于腹股沟疝修补以来，已取得显著进展。但总体复发率无明显改变。前入路修补和腹腔镜修补术后的复发的病例仍在不断增加。本研究报道单孔腹腔镜(SIL）用于复发性腹股沟的治疗经验。

选取年1月至年10月间接受SIL-IPOM方法的复发性腹股沟疝患者。脐下2.5cm切口，腹腔内置入SIL装置。通过“Chopsticks和”inline分离技术，使用单孔腔镜特殊器械进行。在放置抗粘连补片之前，回纳直疝/股疝/斜疝疝囊，或切除精索脂肪瘤。将补片固定在耻骨支后，用不可吸收钉或纤维蛋白胶将腹膜缝合。

共9例患者接受SIL-IPOM手术。平均年龄55岁，BMI26.8kg/m2。平均补片大小cm2，平均手术时间分钟，平均住院时间1天，脐部疤痕长度21mm。在术后20月随访期内，没有腹腔及术后并发症，无戳空部位疝，慢性疼痛或复发。

作者认为，前入路或腹腔镜修补术后的多次复发性腹股沟疝，可以安全的使用SIL-IPOM进行治疗。

INTRODUCTION:

Despiteanexponentialriseinlaparoscopicsurgeryforinguinalherniorrhaphy,overallrecurrencerateshaveremainedunchanged.Therefore,anincreasingnumberofpatientspresentwithrecurrentherniasafterhavingfailedanteriorandlaparoscopicrepairs.Thisstudyreportsourexperiencewithsingle-incisionlaparoscopic(SIL)intraperitonealonlaymesh(IPOM)repairforthesehernias.

MATERIALSANDMETHODS:

AllpatientsreferredwithmultiplyrecurrentinguinalherniasunderwentSIL-IPOMfromNovember1toOctober30.A2.5-cminfraumbilicalincisionwasmadeandaSILsurgicalportwasplacedintraperitoneally.Modifieddissectiontechniques,namely,chopsticksandinlinedissection,5.5mm/52cm/30°angledlaparoscopeandconventionalstraightdissectinginstrumentswereused.Theperitoneumwasincisedabovethesymphysispubisanddissectioncontinuedlaterallyandproximallyraisinganinferiorflap,belowapreviousextraperitonealmesh,whilereducinganydirect/indirect/femoral/cordlipomabeforeplacementofantiadhesivemeshthatwasfixedintothepubicramusaswellassuperiorlywithnonabsorbabletacksbeforefixingitsinferiorborderwithfibrinsealant.Theinferiorperitonealflapwasthentackedbackontothemesh.

RESULTS:

Therewere9malepatientswhounderwentSIL-IPOM.Meanagewas55yearsoldandmeanbodymassindexwas26.8kg/m(2).Meanmeshsizewascm(2).Meanoperationtimewasminuteswithhospitalstayof1dayandumbilicalscarlengthof21mmat4weeksfollow-up.Therewerenointraoperative/postoperative
